By submitting this form, I hereby grant the City of Middleton the irrevocable right and permission to use, reproduce, edit, display, distribute, and publish the photos I have uploaded, shared, or provided (hereinafter referred to as "Photos") for use in social media, print, public relations, press release materials, and other media formats for any lawful purpose.

I understand and agree that:

      1. The Photos may be used in various forms of media, including but not limited to social media platforms, websites, brochures, newsletters, press releases, and promotional materials.
      2. The City of Middleton may use my Photos with or without my name or any accompanying description.
      3. I waive any right to inspect or approve the finished product, including written copy, wherein my Photos appear.
      4. I will not receive any monetary compensation for the use of my Photos.
      5. The City of Middleton is not obligated to use the Photos I provide.
I represent and warrant that:

      2. The Photos do not infringe upon the rights, including the intellectual property rights, of any third party.
